To get things started at Northlight Counseling, please follow these steps:
1. Fill out the “NEW CLIENT QUESTIONNAIRE” found at https://northlightpnw.com/contact/
a. In the therapist preference section, please indicate that you are working with me.
b. After the questionnaire is completed, contact the office at 541-321-2278 or hello@northlightpnw.com to let them know you are transferring
This will help them sort through the many requests they receive in a timelier manner.
c. If needed, clarify for the Salem office
2. Please let them know your current scheduled day and time.
note: if you would like a different time, please check with me as we navigate this transition and I can confirm what days and times are available.
